Case Manager


The Case Manager is responsible for the overall coordination of a client’s clinical and programmatic needs, ensuring that communication between the family, funding source representatives, clinical team and client is open and ongoing to facilitate the flow of necessary information.

Reports to: Psychotherapist


I. Essential

  1. Comply with all Success Rehabilitation, Inc. policies and procedures as stated in all company manuals.

  2. Be familiar with the mission of Success Rehabilitation, Inc. and work towards assuring that this mission is integrated as part of all job efforts.

  3. Serve as liaison between the assigned clients, clinical team, family, funding sources, supervisory staff, direct care staff and the community.

  4. Responsible for on-going contact with families on a weekly, bi-weekly and as needed basis to keep family informed of client progress and any issues that may arise.  

  5. Responsible for on-going contact with rehabilitation nurses/external case managers on a b-weekly or as needed basis to keep them informed of client progress and any issues that may arise.

  6. Responsible for documenting all contacts with families, case managers, rehabilitation nurses, clients or others in a timely manner per SRI procedure and forwarding information to clinical and administrative team members as appropriate.

  7. Responsible to identify and manage client banking needs, including paying bills, review of bank statements, spending money.  Document transactions according to SRI financial requirements to assure all monies are properly accounted for.

  8. Responsible for scheduling client medical appointments/follow-up appointments and informing clinical team and staff.

  9. Monitor DME needs of clients: obtain appropriate approvals; order new equipment; request repairs.

  10. Review incoming mail and determine if follow-up is needed and assure forwarded to appropriate party.

  11. Meet with clients on an individual basis as needed to address need, problem or programmatic concern.

  12. Maintain documentation for all new admissions and insure that copies are forwarded to appropriate clinical and administrative staff.

  13. Update all face sheets as needed and obtain required annual documentation for clients.

  14. Serve as Administrator On-call as assigned.

    II. Non-Essential

    1. Attend clinical team and administrative meetings as scheduled.

    2. Attend external seminars/conferences as requested.


Prerequisites for an individual in this position include:

  • Bachelors degree in field related to health care or human services.

  • Two years experience working with TBI or special needs population 

  • Experience as case manager preferred

In addition, this person must demonstrate competencies in the following areas:
