Position:
Licensed Practical Nurse

Statement of Job:

The LPN is responsible for administering medications and treatments as ordered for clients They assist clients as appropriate in all aspects of daily living, and ensure that the physical wellness needs of clients are met through on-going collaboration and communication with Director of Nursing, Clinical Director, families, outside medical personnel

Responsibilities:

• Administer client medication and treatments as prescribed

• Receive all Physician contact forms, order appropriate medications from pharmacy and transcribe all orders on to client's MAR

• Responsible for appropriate documentation in and maintenance of client Medical Records

• Serve as primary liaison with external healthcare providers and client families on health issues, medical test results

• Evaluate health status and provide first aid as needed to clients and staff

Qualifications:

• Graduation from an accredited School of Practical Nursing

• Minimum 1-2 years experience preferred

• Good verbal and written communication skills

• Ability to work with minimal supervision
