Scope of Services
Success Rehabilitation, Inc. is a post-acute rehabilitation facility specifically designed for adults who have sustained a traumatic brain injury or other neurological impairment. Neurologic impairment, such as a traumatic brain injury, affects virtually every area of an individual's life. Success Rehabilitation carefully integrates its program to provide comprehensive therapeutic and rehabilitative services for both the client and his/her family. Our mission is to help clients obtain a realistic, maximum level of independent functioning in the least restrictive community environment; one that advances individual productivity and well-being.
To provide services within the scope of our program, the following criteria needs to be met:
Have an acquired traumatic brain injury or other neurological impairment.
Be 18 years or older.
Have cognitive functioning that has been demonstrated at an equivalent to a Level 5 on the Los Amigos Scale of Cognitive Functioning.
Be medically stable, i.e. no functioning tracheotomies, no feeding tubes, no I.V.’s, no injectable medications. For persons served attending the Structured Day Treatment Program only, we will accept catheters and feeding tubes that will require no intervention/care during day program hours.
Emotionally and behaviorally stable with the ability to demonstrate control of own actions with no history of excessive physical violence.
Have funding for care.
Be able to demonstrate a potential for progression/maintenance of skills within the structure of a rehabilitation program.
Be able to demonstrate a willingness to participate in rehabilitation program.
Be able to demonstrate the ability to maintain bowel and bladder control with little or no incontinence.
Be clear of all alcohol or illicit substances for at least thirty (30) days prior to being considered for admission and prior to admission for any program option.
It is the belief of Success Rehabilitation that discharge/transition planning is an important part of a rehabilitation program that is person-centered and incorporates an outlook of recovery and resilience even within a long term setting. A discharge/transition plan is developed during the assessment process and is reassessed annually as predicted outcomes are achieved. Planning is done in collaboration with the person served, family/support systems, payers, providers and other stakeholders and considers the living environment and services needed to provide durability of outcomes. If clients’ circumstances or status changes and needs cannot be met at Success, then the Clinical team collaborates with the Person Served, his or her family and other stakeholders to develop transition plan to a safe, appropriate.
In developing this plan, consideration is to be given to discharge/transition not only due to completion of treatment, but to progress within our care continuum.
Should the person served be moving back home, we would provide the family with reasonable support to obtain the services needed in the home. If the person served is discharged to another facility, recommendations for continued services will be made.
Success offers different options for community living in Quakertown, Pennsylvania. Success' main location is a 22 bed residential facility licensed as a Personal Care Home. It also houses our Structured Day Treatment Program as well as our clinical and administrative offices.
In our main residence, all clients have private rooms, and some rooms are equipped with private bathrooms. There are also 6 efficiency apartments (please see On-Site Residential Apartments for more information). The main residence is staffed 24 hours per day. Staff support clients in completing activities of daily living and instrumental activities of daily living for personal care and life skills, such as doing laundry, caring for their room and personal possessions. All residential clients participate in our Structured Day Treatment Program, and social and leisure activities are held in the residence and in the community in the evenings and weekends. Nursing staff are available 16 hours per day, 7 days per week to provide assistance with medication administration, monitor health status and act as liaison with client's private physicians.
Success provides services along the continuum of care, allowing clients to transition through our programs as they progress in their rehabilitation. An individual program plan is developed for each client, incorporating all modalities and based on client needs and goal. Services offered can be provided through employees, contractors or providers in the community.
On-Site Residential Apartments
Success offers different options for community living in the town of Quakertown, Pennsylvania. The On-Site apartments are located at our main facility and offer each client their own small efficiency apartment. Some apartments are equipped with a kitchen, bath, living room and bedroom and some just include a kitchen, bath and bedroom. The On-Site Apartments are staffed 24 hours per day. Staff provide assistance in areas such as meal planning and preparation, budgeting, money management, laundry skills, housekeeping, ADL's, employment assistance, etc. The staff and clients also plan weekend activities to further explore and take advantage of all community options. Since the On-Site Apartments are located at the main facility, nursing staff are available 16 hours per day, 7 days per week.
Off-Site Residential Homes
Success offers different options for community living in the town of Quakertown, Pennsylvania. The residential homes are 3 bedroom homes with 24 hour staffing. Each client has a private bedroom with all of the amenities needed to support an individual while residing in the community. Staff provide assistance in such areas as meal planning and preparation, budgeting, money management, laundry skills, housekeeping, ADL's, employment assistance, etc. The staff and clients at each home plan weekend activities to further explore and take advantage of all community options.
Off-Site Residential Apartments
Success offers different options for community living in the town of Dublin, Pennsylvania, which is approximately 15 minutes from our main facility. The off-site residential apartments are located in the heart of Dublin in beautiful garden apartments. Each apartment is either 1 or 2 bedrooms in which the client has their own private apartment or bedroom. The apartments are staffed 24 hours per day. Staff provide assistance in areas such as meal planning and preparation, budgeting, money management, laundry skills, housekeeping, ADL's, employment assistance, etc. The staff and clients at each home plan weekend activities to further explore and take advantage of all community options.
Outpatient Rehabilitation/Structured Day Treatment
Success services are offered both residentially and on an outpatient basis. Services focus on providing cognitive rehabilitation to improve individual interaction in major life activities including, but not limited to, speaking, learning, working, sleeping, concentrating, reading, processing of information, and communication.
The Structured Day Treatment program is integrated and there is no distinction made between Outpatient and residential clients as far as program services. As with individuals in both Residential and Home and Community Services, Structured Day Treatment is offered at two locations – the Main Facility and Success NeuroRehab, a free-standing facility also in Quakertown. In addition, Success NeuroRehab is the home for all PT/OT/Speech and CRT services for residential and Out-patient programs. The program at each location runs from 9:00 a.m. to 5:00 p.m. Monday through Friday and there are scheduled activities on evenings and weekends. The program is designed to improve independent functioning in these areas through specific cognitive group activities as well as direct instruction, specially designed instruction, goal setting, supervised practice, peer interaction/support, appropriate social skills, technology, compensatory strategy training, self-awareness instruction and self-advocacy training. Utilizing a therapeutic approach, consumers will identify outcome measures based on issues with major life activities and an Individual Cognitive Rehabilitation plan is developed to address these target areas. The plan will document specific cognitive strategies or equipment, which will be used to assist the individual to achieve greater independence in these areas.
Each Cognitive/Structured Day group is facilitated by a degreed staff that is a Certified Brain Injury Specialist with many years of experience working with adults with traumatic brain injury. Lesson plans are generated weekly to outline and implement various daily cognitive group activities.
Focused post-acute care in residential & outpatient settings.
Brain injury affects more than 1.5 million Americans each year, yet no two injuries are alike, nor do they create the same challenges for patients and their families. Our internal care team develops individualized post-acute treatment plans that provide comprehensive therapeutic and rehabilitative CARF accredited services that maximize the quality of care in a residential or outpatient setting.
Brain Injury Services
Our on-site apartments are located at the main facility and offer each individual their own efficiency apartment. Apartments are equipped with a kitchen, bath, living room and bedroom. Others include a kitchen, bath, and bedroom. The on-site apartments are staffed 24 hrs a day.
Our off-site garden apartments are located in the heart of Dublin, Pennsylvania. Each apartment is either 1 or 2 bedroom in which individuals have their own private apartment or bedroom. The apartments have 24-hour team assistance.
Our off-site homes have 3 bedrooms with 24 hour team assistance. Each individual has a private bedroom with all of the amenities needed to support someone living in the community. Team members provide assistance with meal planning and preparation, budgeting, money management, laundry skills, housekeeping, ADL's, employment assistance, transportation.
Our services focus on providing cognitive rehabilitation to improve individual interaction in major life activities including speaking, learning, working, concentrating, reading, processing of information, and communication.

Vocational Counseling Services
To reach vocational goals they have set for themselves, clients have the opportunity to participate in volunteer services in areas such as health services at local retirement/nursing homes, hospitals, local animal shelters and libraries. Should an individual be interested in an area to volunteer that Success does not currently have an established relationship, the vocational counselors will explore new possibilities to meet areas of interest.
