Success Rehabilitation is hiring for a Registered Nurse (RN ) responsible to ensure that the physical wellness needs of clients are met through on-going collaboration and communication with the Clinical team, families, outside medical personnel.

  • Administer medications and treatments as ordered for clients. Liaison with external pharmacy to assure medications are ordered and received from the pharmacy and are correct per physician prescriptions. Assure EMARs are correct and updated as required.

  • Responsible for appropriate documentation in and maintenance of client Medical Records.

  • Monitor and chart client vital signs monthly and as appropriate.

  • Serve as primary liaison with external healthcare providers and client families on client health issues, medical test results.

  • Evaluate health status and provide services as needed to clients within scope of nurse practice acts and facility license.

  • Serve as Certified Medication Administration Trainer after completing approved Department of Human Services Train the Trainer course.


Qualifications for an individual in this position include:

  • Graduation from an accredited School of Nursing

  • Current licensure as RN in the state of Pennsylvania

  • 1year experience preferably in long term care

  • Excellent Verbal and written communication and interpersonal skills

  • Ability to work with minimal supervision in a fast-paced environment

  • Certification in CPR and First Aid

  • Valid Driver’s License
